This is the instrument we used to survey the Rohingyas in Bangladesh and gather information on the prevalence of COVID-19 symptoms, information and awareness, risk exposure, income shocks and food security.

We are providing an example of consent form that was used to switch from in person to phone surveys. We hope this can provide guidance to other projects that are also hoping to make this switch.
